West 17th Avenue Dental Care (“we,” “us,” or “our”) is committed to protecting the privacy and confidentiality of your personal health information. This Privacy Policy explains how we collect, use, disclose, and safeguard your information in accordance with the Alberta Health Information Act (HIA) and the Personal Information Protection Act (PIPA).

1. Information We Collect

We collect personal and health information necessary to provide dental care, including your name, contact information, date of birth, health history, dental records, insurance information, and payment details. This information is collected directly from you, your authorized representative, or other healthcare providers involved in your care.

2. How We Use Your Information

Your information is used to: provide and coordinate your dental care; process insurance claims and billing; communicate appointment reminders and follow-up care instructions; comply with legal and regulatory obligations; and improve the quality of our services. We do not sell, rent, or trade your personal information.

3. Disclosure of Information

We may share your information with other healthcare providers involved in your treatment, insurance companies for billing purposes, regulatory bodies as required by law, and government programs such as the Canadian Dental Care Plan (CDCP). We require all third parties to maintain the confidentiality of your information.

4. Storage & Security

Your records are stored securely in electronic and/or paper format. We use industry-standard security measures including encryption, secure servers, and access controls to protect your information. Only authorized team members have access to your records.

5. Your Rights

You have the right to access your health records, request corrections to inaccurate information, withdraw consent for certain uses of your information (where legally permitted), and file a complaint with the Office of the Information and Privacy Commissioner of Alberta if you believe your privacy rights have been violated.

6. Retention

We retain patient records for a minimum of 10 years from the last date of service, or until a minor patient turns 18 plus 10 years, as required by Alberta dental regulations.
